I was thinking a shield might help. It sounded to me like overactive
letdown. That is the first thing I thought of. If this is the likely
scenario, I recommend almost continous skin to skin like kangaroo mother
care. The mother can try offering the breast after expressing some, giving
baby the hindmilk. If baby won't latch or stay latched, a shield could be
tried if baby is taking a bottle.
Mom will need to be very patient and calm. Of course keeping her supply
going. Positions to minimize the overactive letdown effects on the baby
could be tried. Co-bathing could also help.
