Hello all.

At my hospital, we are far from Baby Friendly and we're having issues with
nipple/flow confusion with our BF mothers and babes. I am trying to get an
artificial nipple and artificial baby milk consent form adopted on the
maternity units. I have the support of the chief neonatologist, and she is
trying to pass it by the other neonatologists and pediatricians. They have
asked me to find out if other hospitals have such a consent form.

If your hospital has a consent form for this purpose (for nipples, formula,
or both) please email me privately so that I can make a list and submit
that with our research.
