The amount of topical bacitracin that the infant would ingest in the
description described would be negligible.
The only theoretical concern would be hypersensitivity in the infant (which
would be even less of a chance if the mother is already using the ointment
and isn't hypersensitive herself to bacitracin).  Signs of hypersensitivity
would most commonly be erythema and swelling.  Most severe hypersensitivity
reactions have occurred with topical use over wide areas or into deep wound
areas or with irrigating solutions, not ointments).
If the baby eats a little ointment along with the milk, there should be no
problem.
