Well, I visited with the mom and dad of the triplets this morning.  They are
36 wks gest. Are receiving oxygen but are not in a nicu.  They have not
recieved any feeds yet when before I left at 10:45am.  Two girls one boy.
Boy's and one of the girl's nurses' is hoping on starting tube feedings
today. the other girl has a tube for oxygen going into the lung as one of
the lungs have colapsed.  If I understand this correctly.
I feel that she was much better encouraged by the time I left.  I taught her
how do
breast massage, nipple stimulation, and hand expression.  I explained to her
that hand expression usually worked better for not only getting the thicker
colostrum out but also for not losing any of the precious drops in the parts
of
the pump.  She immediately understood about losing some of it as she noticed
this when she tried to pump last night.  Mom has told me that the nurses
have been unsupportive ("mean" was exactly how she put it) and even refused
to take and save for her babies what little colostrum she was able to pump
the night before.
Her babies are in the Nursery in isolettes.
I mentioned to the infant's nurse that I taught her to hand express and that
is how we were able to get more milk this time and that she will now be able
to do better at it with as she practices.
The nurse said "but the pump is better at nipple stimulation than hand
expression was". I didn't argue with her but told her that I also showed her
how to do nipple stimulation before expressing.
The infants' nurses said that they intent to bottle feed the babies before
they are allowed at the breast.  I mentioned that I left a large package of
feeding cups with the mother for cup feeds and asked if they have any cup
feeding devices available.  She said, "Oh, no.  We don't do any cup feedings
here."  I asked, "Why not?"  She replied, "None of us are trained at cup
feedings and the babies are more likely to aspirate".  I said that the
babies are more likely to aspirate at the nipple than the breast as well.
She hesitated and then said, "Not if the mom has a strong flow." I said, " I
guess that is one of the reasons why there is colostrum first.  This from a
nurse that calls herself a Lactation Consultant. Well, that is how it went.
To make matters a worse, when she went to the WIC clinic the day before the
scheduled c-section they gave her 3 big cans of Lapase ABM.  She said she
was just wanting the free (medela manual) breastpump because she remembered
when she meet me in class that she should make sure that if she can't nurse
that she start pumping right away.  So, she already knows what the wic staff
thinks about her being able to nurse her babies.
