Hi Ellen,
this is my gut (Pardon the pun ;-) ) feeling about this baby:
I am wondering if anyone has investigated this baby for allergies? Has the
mother tried an elimination diet at all? You do not mention if anyone has
eliminated this as a cause of the food aversion.
Babies are quite smart, as you know, in refusing to eat what upsets them!
Resulting in food/breast refusal in some cases. I believe this often
manifests after the 3 week mark or so, which seem to be the time that things
started going downhill for this baby. Allergic babies also appear to have
'itchy palates' and may bunch up their tongue to relieve it.
Adding Similac to a baby with allergies would not help the situation either
I would think.
Is this baby 'worse' since the Similac was started, as I seem to understand
from your statement?

"During the last few days, daily
    > intake has decreased to 12-13 oz. of fortified EBM. Baby spits bottle
    > out or pushes it away with hands after she has taken only an ounce or
so"

Did the loose stool start with the introduction of the Similac?

I would be interested in what the Gastroenterologist finds.
Do you have a paediatric allergist you can refer to?

Just an idea.
Let us know the outcome.
Cheers
