Dear Friends:
     Last weekend, I attended a workshop with these wonderful OT/IBCLCs. And
only 4 days later, I have a mother/baby dyad in my office. Baby is a bit over
2 months old. Still not gaining weight. Likes to bf in an extended position.
Hates being on her back. Hates the car seat. Wants to bf, goes to breast and
is fine (except for a s/s rhythm of 2-4:1) until about 10 minutes into the
feed.Then pulls off, wails, and wants to go back but isn't happy at all about
it.  When she goes back, her lower lip has lost its tone. Poops once ("dark"
says her mom) per week. Virtually no weight gain.
   I see a tiny, scrawny little girl, with the most worried look on her face.
   After this wonderful workshop and a consultation with a wonderful local
Speech and Language Specialist who wants to learn more about breastfeeding, I
am confirmed in that this is not a breastfeeding problem. This is a medical
problem, probably about some gastrointestinal mobility. Or possibly reflux.
   I would never have known to watch closely for the whole feed. And I
would've missed the drop in lower lip tone after 10 minutes of decent bf.
   I was sorry to see this unhappy baby, and thrilled at the same time to
have use of my new knowledge! I referred and gave a verbal report back to
both the pediatrician and to SLS.
   Wow! I feel like I can be a LC now, after gathering knowledge in bits and
pieces for all my years. I have received a formal and logical structure which
fits every clinical angle I can think of.
