I have searched the archives, but this case has me challenged on several
points.  I would be grateful for any of your comments via both the list and
private email as I am currently nomail.  [log in to unmask]

Mom 25 G1P1
Baby boy born 1/8/03 (gaining weight well, no hyperbili)  Vag birth.
Baby will _not_ latch to mom's large breasts with flattish nipples which
evert readily, but he will latch to the R breast _only_ with a nipple
shield.  However, he will not suckle...mom has the most abundant milk supply
I have ever seen, and the milk pours out, literally.  Baby basically drinks
the milk from the shield and has little motivation to latch on and suckle
since the milk is so readily there!  He seems to need a firmer oral stimulus
to stimulate an initial suck, but will not continue the suck-suck-swallow
pattern.
Mom re-admitted to hospital 1.5 days ago for mastitis.  Is receiving IV
antibiotics.  Mastitis redness is spreading, not decreasing, so caregivers
suspect possible cellulitis.  No abcess.
My question for you: what things can be done to help this baby suckle at
breast w/ or w/o shield in light of mom's oversupply and
mastitis/cellulitis?  She is double pumping after breast bath/massage to the
L breast.
I am considering a consult with a pediatric OT or speech-language
pathololgist for oral issues on baby and I also realize the situation may
improve as baby grows.
