Bernadette Maloney, RN, MSN, IBCLC and myself published an article in the
recent Current Issues in Clinical Lactation on our concerns about early
release of infants with risk factors for poor feeding/weight gain.  It
includes a check list tool that we have given permission to be freely
duplicated.  The tool notifies the baby's doctor that the hospt. LCs or RNs
have observed (check-off list provided on the tool) red flags for problems
that may predict trouble after discharge.  The tool suggests early
follow-up.  While the AAP Guidelines already call for early follow-up (Day
4) for breastfed infants, many are still not seen for 10 days to 2 weeks pp.
This tool (which in Austin is faxed to the pediatrician or public health
clinic of record) puts the responsibility for medical follow-up of the
at-risk bfg dyad back where it belongs:  on the doctor, not on the LLL or
phone counselor/LC.
