Stephanie, stumped, asks for suggestions on where to start.
If there is any member of the team who seems to have the trust of the women
you serve, ally yourself with that person.  It is a good sign that you have
been hired, but it doesn't mean that everyone on the team understands that,
or why.  Concentrate on cementing your common ground with your colleagues.
My gut reaction is 'use the traditional healer' as an informant, about the
cultural context of BF, before urbanization and if possible about the
history of the decline of BF.  Then plan your strategy according to that,
thinking ALWAYS in terms of empowerment (nothing I can buy in a store is as
good as what my body produces).  By your description of the challenges
facing these mothers, almost anything at all would be a step in the right
direction.
Focus on those steps, however small, and don't be discouraged if BF rates
are not 99% after 6 months.  Each mother who succeeds, even a little, will
influence others.
Last but certainly not least, stay on Lactnet and accept support whenever
