We are about halfway through Pride Month in some countries around the
world. I thought I'd share articles about supporting 2SLGBTQ+ with
lactation questions.

2S is a word some queer Indigenous people living in North America use to
identify themselves: 2-Spirited. These people were/are highly regarded in
our communities.

The JHL has a great study in the May 2023 issue and Protocol 33 from the
ABM gives excellent information about how we, as IBCLCs and lactation
professionals, can best give support to those asking.

Induction of lactation in a non-gestational parent has numerous potential
benefits including parent-child bonding, optimal nutrition, and health
benefits to the child and breast- or chest-feeding parent. For transgender
women and nonbinary people on estrogen-based, gender-affirming hormone
therapy, the ability to nourish their infants through production of their
own milk may also be a profoundly gender-affirming experience.

Pregnant people and parents who identify as LGBTQ+, therefore, need access
to nontraditional lactation supports that may be unfamiliar to health care
providers. In particular, barriers to lactating LGBTQ+ individuals include
confronting highly gendered assumptions in the world of
childbirth, effects of transition (e.g., hormonal, surgical), impacts of
induced lactation, colactation, and hospital considerations. The Academy of
Breastfeeding Medicine (ABM) seeks to provide guidance to those caring for
individuals who identify as LGBTQ+.

*Martha Paynter reasons that the parent-infant benefits of
breastfeeding/chestfeeding outweigh concerns about drug regimens that
facilitate transgender women’s lactation.*

Most mothers who have had breast surgery are able to produce some milk.
Some surgeries impact milk production more than others. The last section is
about LGBTQ+ people and ideas on support.
