Hi Erin,
We have 60-70 teen volunteers for our summer camp program.  They go
through an application process that includes an interview and attend a
three-day training.  They create behavior contracts as a large group,
which I transcribe (and add any important things they forgot) and then
they and their parent sign and return it.  We also have instructors
evaluate the volunteers in their class each week, which goes into their
permanent file.  They also fill out an information form that lists
emergency contact and any medication (including the elusive school-year
meds) that they are on.  Let me know off the list-serv if you are
