I think the point with cupfeeding preemies is that, if done correctly, there can be no risk of harm to the baby. However it is possible that sometimes it is not done safely. If the baby is sat upright with its head supported, the cup must be placed with the level of milk just at the rim. The milk *must not* be poured into the baby's mouth. If the baby is ready developmentally it will lap the milk, if not it won't. Therefore, theoretically, the milk cannot get into the mouth if the baby is not ready. The temptation is to push the babies before they are ready. We have used cupfeeding for all our breastfed preemies for the last 2 years, no bottles are used, just breast or cup if Mum is not present. We have had no instances of aspiration, apnea, or bradys during feeds. However staff must be taught how to do it safely. Carolyn Westcott RN IBCLC -- Carolyn Westcott
