I believe informed consent needs to contain the following elements to be
considered as such:

1. A definition of any procedure proposed, the reason the procedure is
proposed, and what is involved in performing the procedure.

2. In addition, a full explanation of any benefits of the procedure as
well as any potential hazards, including unknown side effects.

3. An explanation of all the available alternatives, what they involve,
their respective benefits and hazards, including the alternative of doing
nothing.
