Since 2008, which is a revision to boot, the excellent Toolkit from the
California Perinatal Quality Care Collaborative, freely available to all,
has a consent form for use by known human milk donors (versus the pooled
milk from unknown donors, obtained from a nonprofit milk bank).

It is part of the Toolkit for Very Low Birth Weight babies.  It doesn't
matter if the families you are working with aren't in NICU with VLBW
babies. The consent works for all ages!
