After posting yesterday I went back and reread the text that should have clarified the methodology and realized a very small yet very important word was not included in the paragraph. The corrected paragraph is now live on the site, stating the following: "Since January 2003, these breastfeeding questions have been asked of ALL survey respondents to assess the population's breastfeeding practices." (I added the caps to "all" here so it would be more visible.) I share some reservations about the bundling of vaccination and breastfeeding data together, however, as many things in public health often are, the different issues needed to be weighed, and the option was to continue to not have a nationally representative source of bf data that did not come from a formula company or include bf items into an existing surveillance system that has been validated and established in communities. I am okay with the overall gain achieved with the combination of items, realizing that, like any large system, drawbacks exist.
