Hi Ann,

I suggest you write up a synopsis of what has occured, including
an "incident report" (or whatever term they are using there) and present it
to the risk management department.  I would also inform Pharmacy, as they
are ususally involved when a "medication" error occurs, which this can be
classified as because it was administered or provided to a patient by the
hospital.
