Virginia,
It was a milk bleb.  The skin over the area looks calloused.  It appeared that the corresponding duct continues to back up and cause the mastitis.  I will ask the mother if her doctor stabbed it from above rather than coming in from the side as you recommended.  The infant seemed to have good mobility of her tongue and that did not stand out to me as an issue when I assessed her.
