A couple of things are not adding up for me:
first I'm not sure what a "shelved, anterior palate" means. And I'm not
exactly sure what baby is doing when you say "tongue curl and retraction"
with crying. But I am picturing some abnormal anatomy and tongue function.
That goes along with the dehydration and wt loss. If baby regained this lost
weight by 2 wks, on 95% ebm, I'm guessing mother had a somewhat lowered
supply and needed some formula.
Why then does she have a 2.1 oz intake on ac/pc weight in office? It seems
here that baby got the milk well, and mom's supply is also ok. You also say
baby took 2 oz ebm in ER, again showing that mom had milk in breasts and
baby took it w/o problem.
If adequate volume is taken in by baby, he should gain normally. If adequate
intake is going in, but baby not gaining, or losing, then this is likely a
serious medical problem like inborn error of metabolism for example. But he
showed he did gain weight in the first 2 wks.
But mother says at home he only eats every 6 hrs, and doesn't want to eat, I
dont' get it. But it makes me really worry if mother is purposely causing
harm, like Munchausen by proxy (I'm not spelling that right) or some sort of
EZZO philosophy???? Could it be that mother just does not know how to
position and latch baby and really needs some help with that???
