<a brief survey we can ask bf patients to complete, in order to evaluate a)
our Lactation Services (we have 4 part-time LCs, only 2 of us IBCLC) and b)
bf support at our hospital in general.  Has anyone ever done a survey of
this type?  If so, could I see a copy of it, or could you at least give me
feedback on what you did & how it worked?>

I have been wanting to do the same evaluations as you stated.  I didn't
think about coming to the individuals here.  You are absolutely right about
 "not reinventing the wheel".
With our Lactation Services I work 16 hours a week as LC and have a helper
when I am on vacation.  I make alot of followup calls and have office
appointments and call ins.  How does one know how effective your services
are, how to make the services better, etc. without some form of feedback.

<we found it really hard to track down many moms after 6 weeks.  So when
would be a good time to survey them.  We want feedback on our
post-discharge phone followup as well, so doing it before discharge
wouldn't be enough.>

Ditto!  It is difficult to get accurate or somewhat accurate stats with not
being able to reach these Moms.
I would also like to have a copy or information to be included in a survey
or evaluation.  TIA
