I have been away from the list for a while and have just jumped back in.  If
this has already been mentioned, I apologize.

I might suggest attempting to contact a reputable pediatric dentist.  The ped
dentist we have been using has the ability to not only distract the child but
also give a Novocain shot that is pain free.  She uses the finest needle she
can and warms the med before use.  After using the gel she injects slowly to
eliminate any pain that might be associated with this type of anesthesia.  My
older son broke a lower front tooth when he was about six and never knew that
she gave him a shot!  Our younger son is quite feisty and he also never
experienced pain when having a tooth filled remaining calm throughout the
procedure.  While this was less of an ordeal then an extraction, her method
might be of assistance.  Now if I can only get her to recognize the benefit
of BF beyond a year.....  Can't win them all.
