Hi Mary-Jane,
Sounds like a pretty bf-friendly place there, congrats to those who have
created it.
I am an RN/IBCLC too and I work solely as an LC. However I am hired under
nursing. I take orders and write them on the chart if the doctor has told
them to me. Of course, I then communicate this to the nursery nurse, because
she has the real responsibility for the baby, I'm the consultant on
lactation issues. I regularly take orders over the phone when I'm doing an
outpatient visit in our BF Center (my office), usually for lab work
(bilirubin level) or home phototherapy, and rarely hospital admission orders
for a baby doing poorly.
I don't see a problem with it. You must have an active, unencumbered RN
license, of course.
It doesn't make sense for you to hand the phone over to Nurse Susie, for
example, because the doctor may want to discuss options and banter a bit
with you about the orders. What I usually do is tell the nurse that "I'm
going to be calling Dr. X with some concerns and I'll let you know what
orders he gives, if any."
