Hi!

Our NICN Neo's have decided that our NEC rates may be linked to pumped 
(MOM) breastmilk, (ARGH!!).  They have started to culture milk stored in the 
freezer of "suspect" infants. (signs of "feeding intolerance"). My understanding 
is that they have isolated Klebsiella and Acetinobactor from as least one 
cultured specimen and told the mothers that they could no longer use their 
milk. As senior lactation consultant I am arranging a meeting with all 
the "stake holders" to at least approach this in a more "scientific" manner. My 
feeling is that we need to first get a accurate culture of the moms' milk (as 
opposed to stored milk) Aren't these 2 organisms often associated with 
comtaminants and implicated in poor hand washing? I have searched the 
archives and found some good info. I am wondering what the current best 
practice procedure for collection of a milk specimen is. I remember reading 
about Betadine, and hand expression, but I can't seem to lay my hands on 
that info right now. Other questions I have are; how are other NICN's telling 
moms the clean pump parts, and how are you cleaning the pumps in the 
hospital pump room. (We average about 400-600+ pumping sessions per 
month in our lactation room.) I know that the implicated organisms 
are "baddies" and we need to investigate this further but are any of you doing 
routine milk culturing or have you been involved in a situation such as this? 
We could sure use any input on how to proceed.
