I had a vague idea that I had heard this conversation on Lactnet before. A
search of the archives revealed the following quote

"Dr. Newman's response to the question of how to transition from Reglan to
Domperidone was, "Stop Reglan, Start Domperidone. Easy!""

So I guess that's the easiest of all options. Just switch straight over when
the Domperidone arrives.  I know that some people say to wean slowly onto
Domperidone but I'm not clear on the rationale behind this. I know for
myself that when I took Domperidone to aid in relactation I went straight
onto 90 mg a day and did not experience any problems. Fenugreek on the other
hand was another story- incapacitated me with the runs!
