Pasteurization is a good and reliable option. There are even inexpensive
solar devices for moms to use for pasteurization in resource poor
areas.  Look into "Pretoria Pasteurization" for information.

Anna Coutsoudis did the work on exclusive bf and reduced transmission
risk of HIV.  You can do a medline search to find her work.

Also, Judy LeVan Fram knows a tremendous amount about this issue, since
she is one of our (volunteer) representatives to the UN on breastfeeding
issues.  She's a Lactnetter...
