Dear Lactnetters--
I have permission to post. I am currently working with a mom who is struggling to breastfeed her second baby. Her (aged 5) first was born at 27 weeks and she exclusively pumped for 7 months. Her 5 week old baby is excluisvely breastmilk fed, mostly at the breast; baby does get about 1 bottle of EBM from dad in the evening. Mom complains of severe deep breast pain during let down during nursing and pumping at about 3 oclock and 9 oclock on both breasts. On a scale of 1 to 10, she reported pain at 7. Mom seems to have a robust supply and she reports that the pain is worse when she is full; she had been putting off feedings and/or pumping because of the pain, but she is going to try keeping her breasts from getting too full. Baby presented with a white patchy tongue; mom reports having a white tongue and being diagnosed with thrush since she was a child but says it's never been a problem. I referred mom to her doctor and the baby's ped to rule out/treat the thrush. She says she was diagnosed with MS between pregnancies and has not been taking medication since she became pregnant.
Could the MS be the culrpit of the pain? Any other suggestions?
Thanks, as always, for your thoughts and wisdom.

Amy
