Hello - 

I have a mom who is nursing a two year old, and is looking at medication
options for clinical depression.   The one they are focusing on right now is
Geodon, but most of this class of medications increase prolactin levels.
The mom asks:  I was wondering if this would be something I should worry
about while still nursing?  Also, would it have any effect on weaning, since
that is something that I expect to happen within he next year or so?

My basic understanding is that once milk production moves from endocrine to
autocrine control, prolactin levels do not directly affect the rate of
synthesis or the amount of milk made.  Logically, then, she should not see a
big change in her supply.  Is this accurate?

I also understand that prolactin is measurable in the breastmilk; if
increasing prolactin levels in mom would increase the levels in breastmilk,
what are the implications for her two year old, if any?

Wešve researched Geodon and other meds in this class of drugs, and she is
comfortable with taking the drug and not weaning (but perhaps making
adaptations in their nursing schedule).
