Hi,

I'd love your ideas and suggestions on this case.  A mother recently gave
birth in our local hospital.  She is 33yrs old.  At 23 she had a lumpectomy
on right breast, involved removal of some lymph glands on that side,
radiotherapy and chemotherapy.  She never expected to have a baby.  Baby was
healthy and full term, over 81/2 lbs.  Baby fed well from the left breast in
hospital.  right breast didn't seem to be producing, and mother unwilling to
but babe to that breast. Baby feeding off good breast 2-3 hrly.  Baby had
only breastfeeds in hospital and lost less than 10% and had gained 45g above
birthweight by the time mum was discharged on day 7.  Mum saw dr a few days
later for mastitis and commenced oral antibiotics.  Mum admitted to hosital
on Friday (day 11) with huge swollen painful breast. (the good one, no
change in the other one) Midwives and dr at hospital said they have never
seen anything like it. Mother wishes it would fall off (and says that it
feels like it will)  Was not responding to IV antibiotics after 3 days.
(baby no longer able to feed well b/c of swollen breast.)  mother expressing
with medela pump, but milk not flowing very well.  Not reporting feeling a
letdown.  Mother has now decided to suppress.  Can anyone help us understand
what is going on?  My hunch is that the whole lymph system has been damaged
by the cancer treatment and it's something to do with that.
