You need a simple consent form and Maginnis and Associates has the only
insurance plan for us that I know of.  800 621-3008 in Chicago.

I use the BREASTFEEDING ASSESSMENT AND REPORT  form to send with a cover
letter to the Ped and OB after the consultation.  Ask the netters for Nancy
J. Clark's or Josephine Tullo's phone number or email address to get a pack
of these 4 in one reporting forms.  You can get a superbill "out there", too.
 I use them to pass on to the client for reimbursement when asked.

Please, don't charge a tiny fee.  If you are just starting out..... at least
$65 if you are IBCLC or CLC and know you are good.  And plan to go up from
there.  Believe me we are not getting rich on consultations, and we must make
this service of impressive value to our society both by the good it does baby
and family and by the $ value placed on it.  You can always give a break if
someone cannot afford it.
