I am dealing with the mom who has very hard areola, badly inverted Nipple. When pumping hard to see good streams. She is getting engorged. 3rday post partum. tried pump, areolar softening,lanoline cream. having really difficulty to make baby latch on. see no Nipple tissue at all. 
Is it possible to have few nipple opening on the Nipple?
 
any suggestion?
