I'm not a "pediatrician-type" but can't resist responding to your
post.  Based on my understanding of jaundice:
1. Pathologic jaundice has no relationship to the type of feeding.
2. Inadequate milk intake may slow recovery, but when you're dealing
with ABO, it isn't the cause.  You state that baby was nursing "fair
to well", so possibly intake was not ideal.  If this is the case,
your suggestion of giving EBM which could be measured should have
alleviated this concern.  If an exact measure is insisted on, I
would rather see mom nurse with accurate pre and post feed weighings
and EBM given afterward to meet the "prescribed" amount if
necessary.
3. His approach seems to be based on OLD approaches to breastmilk
jaundice.  I believe it has been shown that BMJ does NOT show up as
early as 3 days!
This doc needs to bring himself into the 21st century!  This really
hits a nerve, because it's something I run into so frequently.  With
some docs there just isn't enough confidence in breastmilk and
breastfeeding, so as soon as there is any condition outside of
normal (and sometimes even within normal parameters!) the knee-jerk
(or maybe just jerk) reaction is "Change the breastfeeding"!
