Subject: Delayed MER

        Second baby, mom nursed first one for 5 months, uneventful for
the most part.  SHe did not want to stop, but because of the babys
hospitalization and her syupply decreasing and him being frustrated, and
getting only "keep trying" help, she quit.  This baby, attached at birth,
7'1oz.  No problems nursing, supply established, then
Day 10 - baby sucks three times, quits, mom offers every 2 -3 hours, baby
repeats the few sucks and falls asleep
Day 11 - Mom calls, I recommend she get a wt. check, then we would
proceed. Baby examined by ped, no problems, wt. is 7"5oz!  Mom insisting
baby is not nursing and she is not feeling her MER.  LC examined
relationship, latch is fine but baby does fall asleep at the breast.
Plan: OFfered one ounce of breastmilk from the bottle - sucked it down.
        Mom got periodontal syringes, and started pumping with Nurture
III and complementing at the breast.  Sometimes after keeping baby at the
breast sucking using syringe, she gets a MER, sometimes not
Day 13 Using scale, mom nurses.  She feels her MER,baby satistied and
tood 118cc.
Home again, some nursings good, some not.  Pumping after feedings, when
she feels her MER, she obtains milk
Day 15 Mom gets scale for home use.  Sure enough, when the mom doesn't
feel the MER baby is taking 10cc.  When mom feels MER baby gets 87 - 107cc.
MER elecited in 6 minutes, 20 minutes or not at all.  No correlation
between when she nurses, if she comps at the breast, what she's eating or
who is there to help.  Mother currently there to assist with 2 year old.
GOod relationship with husband and mother.
Here we are: Baby is ok   ( I was wordered about her ainitially_
Mom has an anorexic personality, is already back to prepg wt.  Has been
encouraged to eat, and has increased intake religiously.
Onno medication,...
THe only "wierd thing" if someone can make the correlation and a
recommendation is she has had a history of kidny stones.  On ultrasound
last week, she has 15 small ones.  Just passed one last night, did not
use any medication, they are small.  Urologist is curious, wants to check
her parathyroid, although in the past, it has been normal.
I'm reaching for anything.  This is very frustrating to us all.  Moms
mother leaves Saturday, I don't know how much longer after that she will try.
HELp, Sorry this was so long, but this is not a case of decreasing stress
or increasing food that can make a positive outcome.
