Thank you for everyone's reponses. I did advise mom she needed a better 
pump however she does not qualify for WIC and I have no pumps to rent at 
this time and her insurance will not pay for a rental so she somehow got a PIS 
new, I am not sure how.

We have been tapping on baby's lip to get to open and he even hates that 
and responds negatively so we relax him and try again after a bit. He is always 
tense, not jsut reactive to the breast as he does this many times with the 
bottle as well. I am looking for a specialist that can see him but have not been 
able to find one in our area.

I will keep everyone updated, for now he at least will lay in moms arms and lick 
at the breast when sleepy..you have to crawl before you can walk!
