Anecdotally only: I've been involved with a case where baby was born just
short of 37 weeks, and was slow to regain birthweight.  Poky at the breast,
being fed EBM by bottle.  As an alternative to the pediatrician's suggestion
to freeze mother's expressed milk for 24 hours, while feeding baby Brand X,
as much as she could keep down (arrrgh), I suggested mother pump each breast
into separate containers, let it stand, and put the cream from one side into
the whole milk from the other, and give that.  This was possible because
mother was producing about twice what the baby was able to consume every
day.  The pediatrician was intrigued, said 'why not?' and mother was
certainly game.

That was on a Friday.  In the course of the weekend the baby gained
startlingly well (big sigh of relief from nervous IBCLC), which also helped
her efforts at the breast.  I didn't have the chance to follow them further,
so I don't know how it turned out in the long run.

I'm following a baby right now where we are trying the same thing.  It only
works if mother is producing more volume than baby needs, which in both
these cases is a side effect of needing to pump due to a poor nurser.  Will
keep you all posted.  Next weigh-in is Thursday.  In both cases these are
babies who are on their way to the breast, but not quite there yet.
