Congratulations on a good article, and the publication!

I'm curious about your "Women who smoke should not breastfeed"
paragraph, however.  The paper you cite is "Woodward A, Douglas R M,
Graham N M, Miles H. Acute respiratory illness in Adelaide children:
breastfeeding modifies the effect of passive smoking. J Epidemiol
Community Health 1990; 44:224–230."  Does that specifically address
the issue of breastfeeding mothers who themselves smoke?  I ask
because the words "passive smoking" in the title generally mean
"smoking" which is the intake of *smoke* from others in the room,
whereas women who breastfeed are directly passing nicotine and other
carcinogens and bad substances to their children via their
breastmilk.

The Inspiraplex study from 1997 states very strongly that the levels
passed on via breastmilk are concerningly high.  In particular they
have found that the level of cotinine in the breastfed baby's urine
is 3x that found in their mother's bloodstream, and the equivalent of
that found in an adult with pack-a-day habit (!!!)
