The Spanish pediatrician Carlos Gonzalez, author of the new book from 
LLLI titled "My Child Won't Eat" and speaker at last summer's LLLI 
conference, reportedly discusses this in detail.  (I haven't actually 
read the book yet, only heard about it.)
