Hi Terri,

Spitting up every meal *alone* is not justification for a diagnosis of 
GERD.  Is this 4-month-old also in apparent distress/pain and often in pain 
when in a lying-down position?

If the baby's main symptom is spitting up every meal, then possibly an 
over-active milk ejection reflex is to blame. Often-times the standing up 
position (that you mention) is best for this and time for baby's gastro 
system to develop further and also for baby to develop techniques for 
controlling milk flow will help this situation.

If baby is not gaining at a good clip, then, obviously, it is imperative to 
ferret out what is going on here.

You mention mom eliminating dairy from her diet, but did she truly 
eliminate all forms of dairy, or at least the "top guns" (milk, cream, ice 
cream, butter, cheeses, yogurts, etc.) totally for a good 3 weeks if not 
longer before assessing effectiveness?  Oftentimes, I have found, mom says 
she isn't eating dairy products, but, when asked further, yes, she has in 
fact had bits of dairy here and there.  For a baby truly sensitive/allergic 
to dairy, a little is all it takes for symptoms to persist.  Dr. Jay Gordon
