About an ounce a day is a good gain to shoot for. Often, babies that have 
lost or have hit a plateau will gain several ounces in one day once they are 
given adequate calories.
What I encounter quite frequently and it is most frustrating is lack of 
followup of at risk dyads. The dyad you describe was certainly at risk and 
the baby should have been weighed (IMO) in 1-3 days post discharge 
(depending on how well he was feeding etc) and then maybe 2x per week for a 
couple weeks to see that the weight gain is continuing. A Tues/Friday wt 
check appointment can work. If a baby finally shows a wt gain at about 4 
wks, and is not seen again until 6 wks, I find fault with that.
