We recently changed our protocol in our Special care Nursery--citing the 2011 publication by HMBANA on human milk storage both at home, in the hospital, and in the SCN.  
(and others)

They recommend 5 days for in hospital, healthy FT newborns, but to make it less confusing, we are using the 4 day rule for all the newborns.  I'm not at work and can't look up the details now--have a broken hand.  HMBANAS publication is fantastic--clear succinct, charts, evidence based with ratings of the evidence.  That is the best resource to purchase for forming policies in my opinion.
