I write this with a mother's permission.After five days of hard work,pain and 
tears,she decided to stop breastfeeding.But the story is: second baby,vacuum 
X4,hematoma and laceration on baby's head.Nursed well after delivery,good 
output,down only 149g by day 2.I was called in(on my day off)just to see this 
mother because of severe engorgement,mom and baby were transfered to 
pediatrics with suspected infection(baby's wound on the head)ruled out. When 
I saw her at 3 days,about 10h since "milk came in",breasts were hard,pain on 
level 10(on a scale from 1-10)She already did ice,cabbage,motrin,pumping-
"nothing comes out",baby unable to latch. She said she is ready to stop now. 
The same happened 12 years ago with the first baby,she ended up having 
mastitis,nursing only for 3 days. So,we talked about engorgement and she 
decided to try again. I used my "russian" method of manual expression which 
combines RPS with massage,and hand expression. We were able to soften 
breasts and latch the baby(for 30 min on both sides),good swallows. The next 
morning when I came back,baby nursed every 1-2h all day and night!,gained 
30g,she kept ice,cabbage,etc.Pain level was about 6 now.However,she was 
almost as engorged as the day before,but with "russian "technique" was able 
to latch baby. This morning engorgement was  about the same,she 
just "couldn't do it anymore". Could she have a problem with oxytocin release? 
What am I missing?She said she would love to find out the answer,we talked 
about stress affecting let down,she was OK with her decision.I feel sad and 
tired,why couldn't I help her?
