As has been pointed out, if even the well-read and highly educated moms
can mess up on mixing formula, how much more must it occur with moms who
are not able to read and interpret what they read.  In pondering how a
Phd, for example, could screw up I realized that we are all used to
mixing beverages for ourselves.  First of all, we rarely measure an 8
ounce cup, we just use our favorite coffee cup or mug.  There's no
precise time a teabag should be left in a cup of hot water.  We measure
instant coffee by "eyeball", not exact measure.  Perhaps the closest
comparison is instant cocoa.  It involves several scoops and some even
say "rounded scoops" which certainly isn't precise.  Now think of a new
mom sho is used to drinking such beverages and is now mixing a powdered
formula (not unlike instant cocoa powder).  It's easy to see how she
might tend to be more casual about the measures and not measure the
water by the exact ounces or the powder by a precise amount.  I suspect
in many cases it evens out in the long run-a little too much powder this
time, a little too little the next.  However, when a mom tends to
consistently over or undermeasure, there is a problem.  It's so much
easier just to "plug the baby in" and not have to worry about measuring
anything.  Just  drink youre cocoa, whatever the strength, and happliy
let baby nurse.
