Anyone certified as medicaid provider for lactation services,  I need help in
filling out
applications. I am RN

 Please email your  email address if you have tried to apply as an RN
provider and can help.  I need to know what classification you applied for to
get a provider number for lactation services and breastpump rentals.

 Ii.e. private duty RN?
