In my country moms who donate their excess milk get paid for it, not 
much but ab. 13 - 15 eur (ab. the same in USD) per litre, and one 
doesn't have to pay taxes on it. It has been done like this for years. 
Donators are tested for e.g. hep B and HIV, and the milk patches are 
tested for bacteria.

Nearly every (!) delivery hospital has its own milk bank, or actually 
we call them milk kitchens. Some hospitals allow moms keep on donating 
milk until the baby is 3 months old, some allow it much longer, like 
up to 8 - 9 months. Some hospitals require a mom donates minimum 200 
ml per day, some require minimum 500 ml per day. Donated milk is given 
to preemies in the first place, but in most hospitals there is enough 
donated milk to be given to other babies as well, if needed.
