Hi Deb B.A physiological fact I didn't know for a long time is that breast development occurs with every menstrual cycle and lactation. Consequently, another attempt can be expected to produce better results than the last attempt. Your niece was able to produce some breastmilk previously so there should be more this time: not 'normal' amounts given the degree of hypoplasia you describe, but more. 
A further consideration would be to begin expressing /colostrum from 39 weeks to create additional stimulation. The goal is to help her feel empowered by what she can do while accepting the limitations of her situation. That is not withstanding miraculous changes to her breasts, which we never want to rule out!
How wonderful for her to have such a supportive aunt, even at a distance.
