If a mom brought in a different brand of formula to use for their baby
that the hospital doesn't use would she also have to sign a release form?

Hi all ~

Wondering if anyone works works in a hospital having a release form for
situations in which parents plan to bring in/provide human milk from a
relative or milk-sharing network? If so, would you be able to share so we
(where I work) don't have to reinvent this wheel?

Thanks!
