I think we've been so interested in addressing the zebra part of this we may
not fully have addressed the horse:

*Is* there an overactive let-down component to this?  Until that's been
really well considered, I'd say anything else - including even
run-of-the-mill cow milk allergy - is a less likely possibility.  For me,
considering it would include observation of a feeding; talking in detail
with the mom about how, when, and why she feeds; asking about dripping,
spraying, and sensations of let-down, and whether she wears/needs nursing
pads; and experimenting with 4-6 hours on one breast, using breast
compressions as needed once breast has softened.
