Thanks to everyone with their ideas and suggestions.
It seems to have been a supply issue: a dwindling supply combined with a
placid baby whose "cues" were misinterpreted by the mother.
For the past week she tried to nurse more often and supplemented with
22Kcal formula after each nursing. Her little guy would happily take about
2 ounces of the formula each time---and gained over a pound.
We are now working on the supply concerns, and will discontinue the
supplements as soon as possible.
