Dear Jaye,
It is not uncommon to see women with a history of a pituitary 
microadenoma who does not make enough milk. It is hard to know why...it 
might be because the breasts are so used to seeing high circulating 
prolactin that they don't respond to the change in prolactin level with 
lactation. That is just a guess.
If her microadenoma is quite small and prolactin levels were never very 
high, I have used domperidone or metoclopramide. If the prolactin levels 
have been quite high in the last, especially with a symptomatic tumor 
(such as headaches or visual changes), I would definitely talk to 
endocrinology first before prescribing these meds.
