This is a case study of a mom I'm working with, and she has given me
permission to post this. Thanks. Please email me privately. The mom is not
pregnant, has not had a period, does not think she is ovulating. She had a
TSH drawn yesterday, has not been dieting, or lost a lot of weight lately,
and is not ill. Kathleen

This is my fourth child, normal uncomplicated vaginal birth. No medication
whatsoever. Baby weighed 8 pounds 5 oz. at birth, latched on immediately .
He was about 1 pound over birth weight at two weeks. At 4 months 1/9/02,
he weighted 17pounds 14 oz.
(All of my kids have been ecologically breastfed, and this has never
happened before.)
I tend to have oversupply/forceful MER, which is now under control.
Jack nurses on first side until he falls off, and will gag if offered the
second side. He poops multiple times a day. I'm not sure how often he
nurses, but I know he never goes more than 1 1/2 to 2 hours.
I'm about a small B cup when not nursing. I stay a full C throughout my
lactation, which is close to 3 years in the past.
The problem: On Monday evening, for the first time ever, Jack drained both
breasts and fussed for more. He wasn't doing much swallowing. I had
noticed at the beginning of that feeding that my breasts didn't seem full.
I figured that my supply was a little low for some reason, so we just kept
nursing. I've been nursing him about every 30 min. to l hour since then,
and have not noticed an increase in supply. He fusses and pulls at breast
w/out much swallowing. His poops have decreased markedly. Although he had
two poops yesterday, they were thicker than usual, w/out much liquid.
He hasn't pooped yet today, but has had three we diapers.
My MER is inhibited quite a bit a times. Sometimes I don't let down at all.
I have been using breast compression and nipple stimulation. I'm not
pumping because he is nursing frequently.
This is the 3rd day that my supply is low. My breasts are saggy and feel
kind of limp. Other than a dull headache and some diarrhea, I have no other
symptoms.
I'm keying off of two main things: His swallowing and his poops. Both
indicate to me that he's not receiving the milk he has been used to for 4
months. This came about quite suddenly, and caught me completely by
surprise.
This is a horrible case presentation, I know, but I'm in a fuzz right now.
A great friend and doctor called me in a script for domperidone at a local
compounding pharmacy. I feel relieved, yet still seek answers...milk
doesn't just disappear!
