Maybe this seems too simple of a solution (besides finding a better and
more caring dentist), but both my *extended breastfed* children have had
or will have fillings/extraction requiring Novocain.  The dentist used a
bubble gum flavored numbing gel on a swab to numb the area first, then
gave the shot.   Also, I was allowed to sit in the chair with my child
laying on top of me while the procedure was done.

You have hit the nail on the head with the comment "it makes the oral
surgeons job easier", yes...that is what it boils down to.  No thought
about the child's fears or ability to cope, just make it easier for the
surgeon.

As for nursing afterwards...I'd just do it, the pressure of the nipple
in the mouth will certainly help slow the bleeding, though it may feel
really *weird* to the toddler since his/her mouth will be numb.  My son
thought it was funny to have his mouth numb and try to nurse at the same
time!!

I'd find a new breastfeeding dentist.
