I work out patient or home health but generally the concern is intake and output/ If no complications are present we follow day 1-1 wet diaper day 2- 2 wet diapers day 3 - 3 wet diapers/ If jaundice or wgt loss is extreme supplementing becomes an issue/ however the nurse should have lactation consultant or the nurse observe and/ or assist and assess the breastfeeding attempt within the1- birthroom, 2- arrival to the unit, and 3-  3 lactch ons or feeds be observed every 2-3 hrs reminder to mom, to put infant to breast( not give a pacifier to sooth) she is the pacifier-infant fdg, intake and output  and log time /offer plenty of fluids/snacks, rest and encourage mom with praise on what a great job she is doing. I would definitely have the nurse assign to patient to send a referral or page lactation consultant as soon as a mom is identified as a breastfeeder. If the mother leaves prior to sucess out patient  nurse  or MD visit next day as well as lactation consultant to follow/ wgt checks at clinic or home health visit. Resources/ breastfeeding hotline / support team- breast feeding coach- (this is the person -usually significant other I identify as the time logger/ personal assistant to  mother offer fluids, rest and support).
